Transition Service for Older Adults

When and where:
Two group sessions per week, 9.30am – 3.30pm:

one session every Wednesday at Boston Road Baptist Church

one session every Friday in the Community Centre, Fieldway in New Addington

Planned key worked individual support on Mondays, Tuesdays and Thursdays

The service will run for two years from autumn 2009
Aim of service:
A temporary specialist service which will focus on older people as a separate group, with the aim of helping them to access appropriate services of choice for older people or other mixed age services. An overall aim of the new social inclusion / day services in Croydon will be to assist people to make optimum use of mainstream opportunities, whether these are age specific or in mixed age groups. It is recognised that, for some older people currently using adult mental health day services, this aim may take time to achieve as hitherto it has been difficult for some people to make progress in this direction.
What will be offered:
Group sessions

Individual support to enable people to use local community facilities.
[New referrals]: Who is the service for and how to access it?
Adults aged 65 years and over, with some flexibility for a small number of people with complex needs aged 55 and over.

People who were using adult mental health day services to Autumn 2009, and for whom an individual review of need has indicated that the Older People’s Transition Services will best meet their social inclusion and recovery needs.

Service users will be referred to the service as recommended by their Individual Review of Need and further referrals will only be accepted from other adult mental health social inclusion services during the lifetime of this service
  To promote, educate, communicate and empower, for the benefit and interest of people affected by mental health issues
